I do hereby give my full consent to be added to the CHS/SON Compliance Blackboard Shell. I further acknowledge that my compliance status will be uploaded periodically onto my personal Blackboard account. I understand that this is solely for my personal use and gives me the ability to track my current compliance standing. I have been made aware that no other individual besides me and the Compliance Office staff will have access to my personal CHS/SON Compliance Blackboard Shell and its contents.
The submission dates of your personal records for the following items will be displayed when accessing your personal Blackboard Account:
• CPR Card
• Liability Insurance
• Community Wide Orientation
• Background Check
• Drug Screen
• TB / TB Chest X-ray
• Tetanus / Diphtheria
• Varicella titer (Chickenpox)
• MMR titer
• Hepatitis B series (3 shots) / Hepatitis B titer
By submitting this request I indicate that I fully understand what has been stated here and wish to continue with the process of being added to the CHS/SON Compliance Blackboard Shell.
